Amaurotic and ophthalmoplegic presentation of Balint syndrome.

چکیده
A 54-year-old woman developed acute hypertensive encephalopathy associated with acetaminophen-induced liver failure. Examination showed blindness with absence of horizontal and vertical volitional and reflex saccades (video on the Neurology® Web site at Neurology.org, first segment). MRI showed biparieto-occipital signal abnormalities consistent with the posterior reversible encephalopathy syndrome (PRES) (figure). Within 24 hours, visual acuity and eye movements improved, but the patient developed ocular apraxia (increased saccadic latency), optic ataxia (impaired visual navigation), and simultanagnosia (inability to recognize more than a single object): the Balint syndrome (video, second segment). This illustrates that a severe expression of oculomotor apraxia can mimic complete ophthalmoplegia and that Balint syndrome may occur at the onset and during recovery from PRES.

[Ophthalmoplegic migraine: case report].
Ophthalmoplegic migraine is a rare syndrome in which episodic fixed unilateral headaches are associated with ipsilateral ophthalmoplegia. Its physiopathology remains obscure. We describe a case in a patient with ophthalmoplegic migraine diagnosed according to the International Headache Society (IH-2004) criteria, who showed an unusual clinical presentation. The first ophthalmoplegic migraine ep...
متن کاملPosterior reversible encephalopathy syndrome presenting as Balint syndrome.
Balint syndrome is a disorder of inaccurate visually guided saccades, optic ataxia, and simultanagnosia that typically results from bilateral parieto-occipital lesions. Visual perception disturbances in the posterior reversible encephalopathy syndrome (PRES) include hemianopia, visual neglect, and cerebral blindness, but Balint syndrome had not been recognized. We report Balint syndrome associa...

متن کاملOphthalmoplegic migraine: reversible enhancement and thickening of the cisternal segment of the oculomotor nerve on contrast-enhanced MR images.
BACKGROUND AND PURPOSE Ophthalmoplegic migraine is a rare condition characterized by the association of headaches and an oculomotor nerve palsy. We report six patients with typical clinical features of this disorder in whom enhancement of the cisternal segment of the oculomotor nerve developed during the acute phase, followed by resolution of enhancement over several weeks as the symptoms resol...
متن کاملNerve-Cell Swelling of the Juvenile Amaurotic Family Idiocy Type: Associated with Tuberose Sclerosis in an Infant Aged Twelve Months.
This paper records the occurrence in the brain of an infant aged twelve months of tuberose sclerosis together with nerve cell changes of a type which, until recently, has been considered peculiar to the juvenile or Spielmeyer-Vogt variety of amaurotic idiocy. Pathological coincidences of this sort in the nervous system are rare and with the exception of Ostertag's (1925) description of infantil...
